This was a game published by Simulations Canada in the mid 1980s. It is a Cold War/World War III game (NATO vs. The Soviet Union) and is focused on naval activity in the Mediterranean Sea and IIRC the North Atlantic around Europe. It is Turn Based and came with a Map with cardboard squares with the names of Taskforces and Ship Names so when you mapping out where your ships are and the other sides ships are (or possibly are) you can keep track of all of it. Both the software and the Map are essential.
Good news is I found the actual SOFTWARE for the game and an Emulator to play the game on my Windows-7 machine. The BAD news is, without the Map that came with the game, it's not playable. This was a game I played in my youth, and when the family "upgraded" from an Apple IIe to an IBM, dad threw out the old computer and all the games that were for it! It has taken me 20+ years to finally find the actual game itself as for years I could not remember the name other than it started with "Fifth" and the Software Company wasn't one of the "big ones" from that time period. So i'm excited to have found the software, but again, I need to find a copy of the map. Even if it's just a screen image that I can design some kind of map and make my own cardboard squares. Any help or leads would be greatly appreciated. Thanks.

It seems like there were reprints of *some* Simulations Canada games by Matrix Games in 2001 and Omega Games in 2005, maybe they have the rights to the game you want now? I did not see your game advertised, but perhaps you can check with them.

I found an image of the game for Apple II, unfortunately out of stock:

But the Atari version seems to have the maps & other docs online:

Since you're not trying to identify a game of which the title you forgot, I moved this to General Gaming.
I don't think I ever before heard of this kind of part boardgame, part computer game hybrid, although it makes a lot of sense back in the days what home computers were not really graphics-friendly.
